The following section provides a tutorial and tips on how to fill out the survey.
Response Forms
Response Forms
•For individuals entering data for 1-3 applications, the survey form is the preferred method of entry.
•Individuals entering data for 4+ applications will be prompted to submit data in an Excel spreadsheet.
•Selecting “4+” or “Yes” will hide the rest of the survey.

Excel Response
•Individuals filling out the Excel form will be asked to provide their background information to track responses.
•Each application should be provided in a separate row.
Survey Tips:
•Please complete the survey in the same browser you started it in.
•You are encouraged to complete the survey in one sitting.
•The survey is mobile friendly, but we recommend taking it at a computer for ease of use.
